  Yes, it's a piece of cake. In the category of the Insert panel/bar forms, select the field to file. UH... that's all.

  No, not automatically. The HTML contains all the features needed to create web forms, but it does not address them. You must use a language server to upload the file to the web server-side. PHP details are here: http://docs.php.net/manual/en/features.file-upload.php.

  Basic PHP functions do not handle attachments. You must use a PHP, like PHPMailer, Mail_Mime PEARor Zend_Mailclass.
