  (1) you don't need to create content for the 6 +. See my next response below.

  (2) you need to create new items for all screen sizes different iOS device you want to target. Other layouts in InDesign can help with this, but you still need to manually adjust everything to get different sizes. It's really not worth it to create a new layout for each different iPhone. It is better to make a set of content 1136 x 640 for the iPhone 5 and later devices and a set of content to 480 x 320 for the iPhone 4 and earlier versions of devices.
