Get (5) different SAS errors for more than a year. Finally noticed after a disk RAID 1 is dead and the replacement drive will not ready. What is my controller?

Just noticed newspapers Windows for my 6/iR PERC have been reporting these errors almost every day for a year, is my bad controller?  If Yes this is very unfortunate because it is integrated.

1.) sense unexpected. SCSI sense data: Sense key: sense 3:11 qualifier detection code: 1: physical disk 0:0:0 controller 0, slot 0

2.) SAS port report: SAS wide port 0 link lost on PHY 0. : controller 0 (SAS 6/iR integrated)

3.) predictive Failure reported: physical disk 0:0:0 Controller 0, slot 0

4.) (ERROR MORE RECURRING): a disk media error has been corrected. : virtual disk 0 (virtual disk 0) controller 0 (SAS 6/iR integrated)

5.) \Device\RaidPort0, restore the device, has been published.

In addition to the Windows logs, what is the status of the disk (VD) virtual and physical disk 0:0:0 in the BIOS of the controller or the OpenManage Server administrator? If the status of predictive failure is constantly reported, then you can consider replacing the physical disk 0:0:0. If the replaced disk is offline, try to force if online via the controller BIOS. Make sure that your BIOS and controller firmware is up-to-date.
