  If you do a search of the forum, you can find the number of threads on your problem. Have you tried to remove the upper/lower case filters to the registry? Try to use high quality media like Word to Word and Tdk, clean with a lens cleaning disc. It's typical responses to your problem. If it does not help, it may be hardware issue.
