System claims the item from start menu pinned available or moved without reason

    For me, the name of the file to be deleted in the %AppData%\Microsoft\Windows\Recent\AutomaticDestinations was 9839aec31243a928.automaticDestinations - ms, but the name can be an id and opposed on various systems.

    The best is to find your file reorganize everything, PIN or detach a file in Excel, or just to open and close a file and then search for the file with the most recent date any time.     Pavilion dv4-2145-dx Windows 7 Ultimate x 64.

    I install a new program that puts an icon on the desktop.  I right click on the desktop icon and select Pin to the Start Menu.  When I click on the Start Menu, the icon has been converted with a default Windows icon.  By clicking on the appropriate program icon default access.  Right-click on the icon/properties by default displays the correct icon and if I click on change icon the icon displayed is the correct icon.

    Any ideas as to why this icon is converted into an icon of windows by default and how to solve this problem of icon?
