  I fear that the only way to get this computer secure and fully patched to Windows Update will be formatting the hard drive - and then do a clean install of Windows.

  Since you don't have the disks that originally came with the computer (and assuming that the original owner would have given them to you when gave you the computer if there were), we must assume (or at least to cross our fingers and hope) that there is a hidden restore partition that could be used to return the computer to the condition it was before first it left the factory.   Currently Windows Vista Home Premium SP1 x 86 (no patches since 2008). Currently, it has no AV and has not been connected to the internet yet. It will be behind a router.
