  As the creation of widgets is an echosign feature and not an Acrobat feature, unfortunately, you wouldn't be able to create widgets with the Acrobat DC pro subscription.   When you create a manual widget it create no menu items based on pages in plan view.

  Once you select 'manual' in the type of menu, it will give you 1 single menu option.

  

  Once you click it again, then it will you give '+' sign, right to left, a bottom. You can use the bottom to create submenus and then bind it with any page in the Plan view.

  There is no such a model, so you'll have to make your own, using a spread on presentation of your newsletter.

  Use shapes, images and text boxes to position available for your model.

  You didn't say what version of Pages that you are using but if it's Pages 5.x, it has no text boxes related so this will make it difficult and it's going to be a lot of manual work.
