How to create text with a bar full of color boxes at the top of page 1/3?

I am in the design of learning and using CS4

I want to create a form with boxes like those below.

I understand how to set a block of text on top with white text, but how can I box behind that bar (incl. rounded corners) color

Once I have the design that I was intending to use the Adobe PDF software to use their tool to have a PDF form.

So I would ask as I go about it in the right way?

It is a multi-step process and can be done in a number of ways. Here's one way:

Create a rectangle and rounded a corner. Then, create a column once, two-row table. Fill the upper cell with the blue wire. Make sure that it is a little wider than the original rounded rectangle. Cut the table to the Clipboard and then select the rounded rectangle frame and choose paste into.

That should hopefully help you get started.

    Unless you have installed the full software from HP support features, none of the options on the cartridges of ink or HP Diagnostics will work

    My C510a eStation printer does not print using printing applications.  Only, it prints a thin line at the top of page.  Everything works fine until I feel.

    All other functions seem to work.  It prints computer via WiFi.  It prints when I send an email to it.  It copies via the Zeen.  It does not print using printing applications.

    Does anyone else have this problem?  What is the solution?

    2010-12-23 15:00.  I just got got off the phone with a HP engineer.  She said that the station is still in TESTING phase and to call to report problems, allowing them to improve the product.  (I thought that the product would have been fully tested before being released to the public!)

    She had me reset the zineb display by pressing the volume keys during power on.  This erases everything that had been done on the Zeen by me and gave rise to no HOME screens, which will need to be implemented by me.  The only application at the bottom of the blank home page was Yahoo!.  The app of the printer was still missing.  I had to move the tray app home page.  Now, I have to spend hours, once again, setting up the Zeen.

    After the reset, the printer apps seem to work now.  The Zeen will copy by using the application of the printer.  My computer seems to still be able to print and scan to/from the station.

    The printer station eStation C510a and zineb display are very exciting.  However, they are not for someone do not want to spend the time necessary so that it can work properly.  If you have a cell phone, be prepared to use a lot of minutes while on the support.

    The HP engineer said that soon will be a new version of Android on the Zeen enabling it to be faster.

    Using the layer style salt described in the link of the Peru Bob, here is a step by step to create this effect:

    11 move the text layer duplicate to the bottom of the stack and repeat steps 3 through 10 with this change alone:

    The scale of the effects use a larger scale for the Red spark.

    Later change a color sparkle, ctrl-click that layer and add a Hue/Saturation adjustment layer above it.
