  As you use index 2 your second check box, you must use a different number for the item DISPLAY_AND_SAVE - if no other columns are editable, so you can use 3.

  An important point to note is that only checked boxes are submitted with the page - all those disabled or unchecked are completely ignored (it's a browser thing not an Apex one). This can have major problems when you try to connect the boxes checked with other values on the line. If the user has checked only two articles about 15 ranks, F01 would only have 2 values but F03 the full 15. As the boxes return the EQUIPMENT_ID value, you may be easier get your PL/SQL code to retrieve the DELIVERED_SERVICE_NAME value directly from the database.

  DAQmxSetCOPulseTerm() only accepts one signal, but DAQmxExportSignal() accept multiple signals, so here's a simpler way to do the routing:
