  In "gone", I assume that you do not see the menu of all the Programs.Accessories? Try clicking on the Start button and typing sound (or tape recorder) in the search box. The sound recorder.exe file should then appear at the top of the search list.

  This toolbar is common to Dell systems, called a Delldock, it is not part of Vista, but rather is installed by the manufacturer. Other manufacturers, such as HP, have similar tools.   I did it for myself once. It tuns out I somehow put in with another icon. Appearantly some icons can be added to others - as what makes a subgroup within it. Can't remember what it I put it in tho, sorry.
