  What version of Windows are you using?  If you have Windows XP, I'm afraid you're stuck.

  An iPhone IS requires at least iTunes 12.3.3 which in turn requires at least Windows 7.

  12.1.3 iTunes (from your tag) is the latest version running on Windows XP (which is what I suspect you have).

  If your old phone has a restriction code, the restriction password is saved in the backup.

  If you don't know, or want to remove it, you have no choice but to start as a new phone 'break the chain'.

  According to what restrictions are defined, you can restore this backup anyway... to synchronize your data manually or to iCloud and then reset the phone like new.

  Use iCloud, simply join the Mac and iPhone to the same iCloud account and turn the services you want on, calendars etc.

I want to replace the iTunes folder on the new Mac with my backup and everything on the new Mac to work as it did on the old, well that the old iMac and a new have different names, I do not know if what counts.
