  Basically, you would need Acrobat to insert pages in a PDF file (unless the creator of the PDF has provided a mechanism to do this with the reader).

I now need to produce another form of web, but one of the questions must have a drop of water in a box - working example ( http://dt7design.co.uk/Forms/Form.php) and the coding does not work for this section - nobody knows how this code correctly.   Looks like you need to add values for the "select" menu options. So, more like:
  Green

  The label is just in the interest of the browser (in general), and the value is what is presented to the script. From there on, the treatment would be the same in PHP (unless it's a multiple selection menu, you must manage the tabular value)

  I ran into a small issue and cannot see what is causing my email form does not work. I did a few times now and that you have copied the actionscript code that I currently work but the it still does not work. The form can be seen live here and the flash file can be downloaded from here.
  
  I am enclosing the code to my flash below form and if anyone requires the code to my page email.cfm (this is the page that processes and sends emails) so please let me know. I'd appreciate any help I can get with this, and I work on it for a few days now and have rewritten the code about 6 times - it starts to do my head in.

  I have a little problem with the AutoComplete feature in Adobe sending / track.   My problem is when a user type a wrong email address has no way for me to get rid of AutoComplete.   I tried tools Erase everything and nothing seems to work. Erased everything in the browser frame and still does not work. Now we have 10 different versions of an email as and 9 of them are wrong because of typo on each of them.  How can I delete the incorrect email addresses in the AutoComplete list? I've looked everywhere and there is no option for this.

  This feature is useful if the user could clean/remove e-mail addresses of disorder, this way the list will not show incorrect addresses when 9 of them are completely wrong.  But for the moment isn't an easy to use and a very frustrating option.

  Can provide you instructions on how to proceed if there is?

• Basic PHP email form does not work after upgrade to 20141

  I have a MAC OS X 10.7.5 - I have 2 sites that use e-mail very basic PHP forms that worked very well until I upgraded to Dreamweaver.  Now, I can fill in the form and there is not error - it seems that it works as before, but I never have the email.  Says Adobe support has nothing to do with the upgrade and there is an error in the script code.  Can someone help me?  Here is the send.php

  Sure that you will find the upgrade is not to blame, and the timing is a coincidence.

  Cause the most likely problem is your SMTP server being modified settings. It is very common to block mail scripts because they are a major source of spam.

  In addition, as Rob says, always include a "From" header It is not required, but doesn't it have will significantly increase the chances of your anti spam mail.

• Sign and email form without a no save option?

  It is not possible that a document cannot be signed without being saved. Even if it were, according to the Acrobat license agreement, you cannot enable documents to others outside your organization to use to collect data. What you can do is to create a form that sends only the data form by e-mail, but it would prevent users to sign. The recipient of the data can then use Acrobat or another program that allows data to be imported into a blank form and save it.

  If the signatures are required, the person who needs that must get Acrobat Pro and activate the form. There is a limit of 500 returned forms they can use data if the active document is distributed to more than 500 recipients. You can always create the form for them and to provide a version that is not compatible.

• Allowing users to add additional lines in a PDF form?

  I use Adobe acrobat X Pro to create PDF forms to capture billing information. Yet, I can not understand how to allow a user to add extra charges. I could create a form that just had a large amount of load lines which could be left blank, but was hopeing to allow a dynamic form of setting where someone can click on a button to just add a new entry of line in the form and the form would settle down and enter in a new series of line entries. Is it possible to do it in Acrobat X Pro?

  For the foorm to automatic adjustment in response to the addition of a new line, you should examine what can offer a dynamic XFA form that you create with LiveCycle Designer. LiveCycle Designer ships with Acrobat Pro as a separate application.   Line 37 is the following:

  Also the From header: is: $headers = "From: $email\r\n"; but I do not see the variable defined for $email

  Also, you need to know about electronic fuel injection. Someone could inject your headers and add additional BCC: addresses to send emails to multiple recipients undisclosed through your mail script. You would use your script to turn your server in a host for spam! Sanitize your fields to prevent injection attacks! Safety must be the priority of the followed firt of functionality. If it's not safe, it should not work

• trying to create a contact email form using php script

  I have a contact form in a flash animation. The form acts as row fields 10 1 and 1 comment / multiline box. I am a naked extreme newbie with me. In the contact form flash I have a button "submit" with the following action code related to the it is attached.
  
  
  The commented code is my attempt to ensure that some fields are filled in by the end user before they click on submit, but this is another issue I will deal with time - I get the form to send an email.
  
  The php file that is supposed to send the mail is also attached. This doesn't really work. I have no idea why and I read tutorials and Web sites throughout the day. I'm about to buy one of the forms of presentation flash $6, but it would really rather than mine works. Any help would be greatly appreciated. I'm to do it in Flash CS3, but using AS2.0.   I realized my problem. I did not understand the thing about the entire property and text fields have an instant name and a veriable. You learn something new every day, I change the snapshot name and did not even know the variables. Wow things allot easier now!

  I am considering buying a Blackberry Curve (Sprint).  My hang up so far is that I want all my emails sent my Outlook on my computer at home.  So far, my solution was to use GoToMyPC so that I can connect to my PC at home from any computer and take care of my emails.  Also, I work mainly at home, so this isn't a huge problem in terms of convenience and efficiency.

  Most people tell me that they themselves CC when they send an email of their Blackberry.  It's how they keep track of important emails sent from their Blackberry.  IS THERE ANOTHER WAY?  A MORE RELIABLE WAY?  IS THERE A WAY TO HAVE ALL EMAILS SYNC'D WITH OUTLOOK SYNCING (THEORETICALLY) MY BLACKBERRY WITH MY COMPUTER?  IF NOT, WHAT IS THE BEST WAY?

  I have 2 two e-mail addresses; is the address of my blackberry, and the other is my Outlook.  For the most part, I give my Outlook address so I could have on my PC at home, and when Isync it is there. Or, in the field if I inadvertently delete a message, I know that it will always be on my PC at home. I think your friend is correct. If she has the Blackberry

• Creating an email form multiple group subscriptions

  I am trying to create a landing page that will allow users to subscribe to several e-mail groups (such as the page list of subscriptions, but with a field to enter a new email address and language on subscription rather than unsubscribe), using check boxes to select the groups to which they want to subscribe to.

  I created a form with the fields, choice and e-mail list, but I don't see how to subscribe to a particular group e-mail based on the value of the list of choices, as the only way to make a conditional update of list of choice is to set the value to 'exactly' or 'not exactly' value.

  Basically, I did a work around, creating a form with an e-mail address field and a custom field check box for each newsletter.

  Then, I created a processing step to subscribe the user to each group of newsletter. For each of these steps, I put the condition that the corresponding field of the checkbox "match exactly" on.

  "" For example, newsletter Previews that we have, I created a custom checkbox field called "Previews" and implement processing step to add the user to the Subscriber group Previews when the field Previews "corresponds exactly to" on.

  I Don t care what my hosting provider tells me but e-mail from contact Muse forms are NOT sent to a few sites that I sold to my clients.

  Occasionally (a few times per year) this question appears, due to some very strict and more anxious hosts. They occupy a point in this possibility of abuse on (which can be avoided), but there are some methods more 'old fashioned' , which are always crucial in support. However, if this is their final verdict, then read in this thread (from the starting point of the link) and find someone who can do some coding like this for you.

  I've set up a contact form on Muse pointing to my email address [email protected] .  I use google apps for businesses to manage e-mails from my company.  I get the message error "the server has encountered an error.'

  
  Here is the really strange part.  ONLY, I get this error when sending a [email protected] e-mail address.  Any other e-mail address goes through but not addresses ending with gmail.com.  Please please help.  I tried Googling, read the article that you have provided and nothing is explain this.

  If I'm not mistaken, notification form is not received only if the address email @gmail.com is used in the submission of the form?

  If this is the case then please contact your host and let them know that @gmail.com addresses are filtered, in the case where miss me something here, please let me know.
