  Both programs were given to merge. InDesign would probably work best for what you want to do, but you might be able to get away with PS:

  I see this a case of duplicate field names. For example, in a pdf form, you have 2 text fields. If the names of these two fields are same then it will duplicate the data as well. In this case, please go to the editing Mode form using Acrobat DC on your PC/Mac and give different names to the fields.

  According to my way of thinking, you want to export the file details of all your files on your HARD drive to an excel file or a txt file. If I'm right in this case, it is not possible to export the details of each & every file in the excel sheet.

To convert this into a single line, with the new columns we could simply use CASES or DECODE in the select as the SQL below.   When the user clicks on the radio button, the value of the multiline text box is displayed in another text box.
