iTunes 12.3 mess up tags and covers my music

I already had problems of this kind trying the music free Apple 3 months in July, but I could reconstruct what I messed up. Have had no problems since then until now, with iTunes 12.3

It's worse than ever. This occurs for about 1 song on 8. I hear a song and in the moment, the song begins to play, the coverage goes away (from the song and the album), tags (artist, composer, Label, etc.) have been changed and I don't know why and what to do.

I don't have the music of Apple, iCloud, music game iTunes library or activated iTunes in the cloud. I also disabled "share details about your library with apple." So yes, in fact that I use my library of music completely offline, as I always did, but the problem still remains.

I tried to restore everything with an old file to iTunes library, but that did not help.

Any help? Any idea? Please, I beg you!

I'm done with it. Too good is messed up, too many labels have been rewritten, too blankets disappeared, changed too much. I can't restore it via an old iTunes library.itl file.

I have only one hope: my music library complete (with all caps) has been synched my iPod touch until I installed iTunes 12.3, so, is there a way to remove the existing library and to restore it with the contents of my iPod touch?

Of course, I would like to hear other ideas that could help me.
