  Hey, JSammy.  I had mine works again with the help of someone from another site.  Here's what you need to do: press the green call button, then the menu button.  Select options, then on options General, highlight "dial from homescreen" and the value. Save and exit. Now on your home screen, press "O".  After that, the General menu options will come.  Choose the option 'theme' and choose a different theme or return to the default value.  This should be all your application icons to reappear.  After doing all this make sure you go back to reset the "dial" since the homescreen on 'yes '.  I hope it works for you.

  I've read tutorials on how to make a form, but when I go in the menu 'create' and I try to start with "Create a form" nothing does, I just a list that says "Adobe create PDF" and how to export PDFs.   First of all, download and reinstall HP Quick Launch Buttons on the link below.
