  I know this isn't much of an answer or even very applicable but everyone I know eventually loses their emails from mobile provider or cable operator sends cause unless you stick with them forever, your account is deleted after you leave them.

  Gmail, yahoo, rocket mail, and even AOL don't do that.

  Dreamweaver is a front-end development tool. Databases and processing of forms are back-end development. You can certainly use DW as your environment to work with backend scripts, but it is relatively unimportant to the main development workflow.

  So, find a course on using PHP/MySQL. It is not really important if you work with DW or another IDE.

As you can imagine, a person who is order from them would most likely variations plus a print order if will have to answer the same entries in form again and again. Now my original idea was to create 1 massive form with 60 different entrances, but I realized that it would create a huge headache from my client side. Especially with BC lists how their web form with 1 row on time results. Does anyone know of any ideas on how I could create a complex web form that would allow a customer to complete an order (similar to the picture below, but translated digitally) and have it present in a manner as well?

  We have recently made our website with Muse and successfully transferred to our hosting provider. We are stuck with not be able to put a simple contact form. The Muse tool is so powerful and is supported for the non-programmers like us. Amazing!, but stops short when it comes to the shape. Do not understand this!
