What should I back up whenever I install an app from iTunes?

I am curious and annoyed.  If I connect my phone to my windows computer and start iTunes and to install an application that I have already downloaded on iTunes on the phone - why should the phone back up first?  Is it possible to keep backup and install separate applications?

If you back up to iTunes it will want the backup device during the first synchronization of each calendar day. This is an incremental backup and generally not long to perform. You can not turn the behavior off, except by switching to iCloud for backup.
