  The behavior is the same in Acrobat 11 as well. The thumbnails do not get updated until you close the document and reopen it.

  you would use ID at all that will change your layout and you don't want that. in this case change all your refferals in a label - anchors

  and then in the CSS position them based on the resolution in a relative path:

  You might have a page element outside the margin to the right of the page area. Zoom in on the right (Ctrl + 0 / Cmd + 0). Then select all elements on the page with Ctrl + A / Cmd + A.  Check if there are stray items.
