Add line button adds three lines (not one)

In the report of Dropbox form - UNIFORM - (04) .pdf there are two tables. The two tables have a single button "add a line". In the table at the top of the page, click it once actually adds three lines (not one, as it should be). In the second table, it works as it should (by adding only one line). However, for the life of me, I can't understand why.

The upper table using this code:

Except for a form of corruption of document, I can't begin to guess why the top adds three lines, but inside only one. It drives me crazy.

    It seems that your screen displays some graphical anomalies when you connect to your user name. I would use these troubleshooting steps in the following article on the resolution of screens:

    Restart your Mac in safe mode to reset the default display resolution.

    If booting safe mode doesn't resolve the problem, reset your Mac NVRAM and SMC to reset the video ports on your Mac to their default values.

    There may be a problem with one of the files which stores details of the installed extensions. Try to delete these files as described in the section "Corrupt extension files" of this link - https://support.mozilla.com/kb/Unable+to+install+add-ons

    Although this article is not specifically about your problem, the method indicated in the corrupted files extension section can help. When you restart Firefox after you delete these files, the list of installed extensions is rebuilt and cela hopefully solve your problem.

    You need not install older versions in order to get the last update enabled.  During the CS4 activation process, you can specify the eligible previous version (choose the oldest among a list, then enter its serial number).  Choose any one you had just before CS4.

    If you don't have a heading row in a table in your worksheet, you will not be offered the creation of a new form option. What is happening in your worksheet?
