  This issue is finally resolved. After I compared the FORMS_BUILDER_CLASSPATH variables in the environment vs registry Variables, I found I was missing C:\Oracle\Middleware\oracle_common\doc\online_help\oracle_forms in the registry. After that I added it it still didn't fix it. Then I found that entry C:\Oracle\Middleware\Oracle_FRHome1\jlib\ewt3.jar was pointing at my old House, so I have fixed to point to Oracle_FRHome instead of Oracle_FRHome1 and my forms launched immediately.
