  Once you have completed the form, you can use Email (next to icon print at the top) icon in CD player to attach the PDF form by e-mail.

  Or in the case where there is any option button Submit the form so click it and it will take you with the workflow of the sending of this form.

  Last option is to save the form and open your e-mail like Outlook, Thunderbird client and attach the saved PDF form.
