Open my libraries predefined defaults to files instead of en_US en_US, and when I create a new documents they are created in British English, although I chose American English in my preferences.   Some files preferably holds your previous value. You can try to search preferences and deleted. You can try to uninstall (disable all first to stay on the safe side). Then use the clean script: http://www.adobe.com/support/contact/cscleanertool.html. Then check it twice:

  Go into Library/Preferences/AdobeSystems ditch this

  Go to Library/Preferences / (ditch what you see there) and all the com.adobe. plists.

  Come on in/Library/Application Support / ditch everything you see related to Adobe (Adobe, Adobe Systems, Adobe PDF).

  The repetition on your library user, ~/Library/Application support ~/library/preferences folder.

  You can now delete anything and everything about Adobe. Then you can re - install and re - activate. All in all an entertaining afternoon. Something I did recently to get CS 5.5 installed correctly.

  Have you tried plugging the headphones into an output a few times?

  Does say "headphones" when you toggle the volume and no headphones attached?
