  I upgraded to v37.0.2 and now the toolbar above (the one with the URL, search inside), the new icon on the top toolbar - extreme right (it looks like server).

  This looks like a problem with an extension.

  I found the answer myself. In Firefox, go to tools, click Add-ins, click Extensions and click the Remove button to remove the following extensions:

  Searchme, and
  Slick saves

  The only time I see two icons for a Mozilla application in the dock is when I have to restart after installing an extension or a new theme. The additional icon disappears if I restart again. I have an extension that gives me a reset button to use. There is an add-on for Firefox that can give you a reset button, if you care to try this remedy.
