  I think what happens is that when the device is initializing, DHCP tries to get the IP address to the VLAN 100 native. After that CDP discovers there is a voice VLAN, endpoint application address DHCP VLAN 11. So you see twice MAC:

  I would say "try and do a restore as new iPhone" like everyone else but I'm having a similar problem and nothing works! I've restored all the possible ways and nothing. I got my screen replaced and I still have the same home button but my phone updated with error 53 (makes the phone unusable) but the camera did work before the update. and Apple has released iOS 9.1.2 (13D 20) to correct the error 53 (mainly for other reasons) but when I go to use the camera, it is only black!   I ordered the Dell XP disc and did the re-install. It works fine now. The Dell technical support had me do some updates on the web as well. I have not had problems since. What happened with the automatic updates completely wiped out my operating system. At least I don't have to buy a new computer.

  If nothing else works, try reinstalling your operating system. It worked for me.
