  I have confirmed that your MAC is correct. The problem seems to be the KCV. It's the first 3 bytes of the last 3. I have not checked to see if there will be additional problems after that, but fixing that can solve your problems.

  If everything is OK and right drivers all USB ports should work. You cannot install the drivers for each port separately and force one of them to work properly. On this machine, all three ports are the same USB2.0 ports.

  Try reinstalling the chipset utility.
