  Edit: it occurs to me that you can also have suggested active apps. Settings > general > transfer & suggested Applications. Cut the two settings and see if that makes a difference.

However, now they are completely empty print - and only the content that is typed in the fields print.  Usually I create the form in Word template, save it as a PDF, then PDF Pro, I use the "FormTyper" option in "forms" - it automatically creates the form - I make changes (as the deletion of the text boxes where didn't need, expand a text box, etc.), and then I am able to print the blank form (print only the basic information) , or I am able to type in the fields and then print the full form.  Like I said before, now nothing prints except what is written in a field.  Please, any help would be GREAT!

  You can not attach the Valueset to the Lov directly. You must create a VO for this.
