  I found the problem with your form...

  (1) you must save your form as the form XML Dynamci. (File-> save as and choose dynamic rather than static).

  (2) in the file-> properties of form tab by default, at the bottom, to make Format PDF, you chose, static XML form. Change it to dynamic XML form.

  (3) in the same way under the Preview tab, you can change the dynamic static XML form.
